Note from IL: The manuscript originally published on this web site in mid-2002 and then made available on CD was substantially different from that now being published. A whole new chapter ("A Spiritual Worldview") has been added, and there are multiple other changes throughout that, if not substantially altering the main thrust of the arguments, certainly provide important new evidence in some cases and change the emphasis of the arguments in others. I strongly recommend that any serious researcher should make sure they read the newly published edition, and can only apologise that this necessarily involves a doubling up of effort.
